29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    (a). Worksite located at 111 S. 6th Street, St. Joseph, Missouri- Employees had climbed out using the guardrails of the aerial lift in order to access the interior of the building.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:     (a). Worksite located at 111 S. 6th Street, St. Joseph, Missouri- Employee was exposed to a fall hazard in that the employee was observed working in an aerial lilft without the  use of a harness and lanyard connected to the boom or basket of the lift.
