29 CFR 1910.30(a)(1): Portable and powered dockboards shall be strong enough to carry the load imposed on them.    a- 99 Newburg Rd., Hackettstown, NJ, Loading Dock facing the back parking lot near the entrance door - The dock board was not in proper operating condition and not taken out of service until repairs were made.     Condition noted on or about 11/21/13.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a- Preferred Furniture Service - Employees who are required to operate powered industrial trucks were not trained in forklift operations.      The employer must also certify that each employee was trained and evaluated.  The certification is to include, but not limited to, the name of the operator, the date of training or evaluation, the name of the person providing the training or evaluation.    Condition noted on or about 11/21/13.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    a- Preferred Furniture Service, Hackettstown, NJ - The OSHA 300 log or equivalent was not maintained by the employer.
