29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  East, repair department, Wilton horizontal bandsaw: on November 21, 2013 employees cutting tapered wire rope sleeve pieces were exposed to amputation and caught-in hazards where the unused portion of the blade was not guarded and a drive flywheel guard was missing.  There is no abatement certification or documentation required for this item.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  West, wire rope department, Hyster S150A propane-gas powered sit-down lift truck and E035 electric high-reach lift truck: on November 21, 2013, authorized forklift operators did not receive required performance evaluation at least once every three years to assess their level of competency in safe operation.   There is no abatement certification or documentation required for this item.
