29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, aisles and passageways had obstructions across or in them that could create a hazard.  On or about 11/20/2013, the employer did not ensure that aisles and passageways were free of obstructions that created potential tripping hazards.  INSTANCES:  a)    Three inch angle iron was bolted to the floor around each operator position next to conveyor belts in the receiving area.         The angle iron was not painted in a noticable color to alert employees of potential tripping hazards.  b)    Eight inch high ballards were cemented into the floor and located at the end of each conveyor belt in the receiving area. The ballards were not painted in a noticable color to alert employees of a potential tripping hazard with the possiblllity of falling onto the moving conveyor.

29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked:  On or about 11/20/2013, the employer did not ensure that permanent aisles & passageways used by powered idustrial trucks throughout the distribution center were not marked to denote safe passageways for traffic. Throughout the distribution center, passageways were shared by both vehicular traffic and pedestrians and the placement of pallets containing inventory were placed in such a manner that restricted the flow of traffic in some areas of the warehouse.
