29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Location: 449 Boston Post Road, Orange, CT 06477 Each employee required to operate the Clarklift Model TM powered industrial truck (forklift) had not successfully completed a training consisting of a combination of formal instruction, practical training and evaluation to ensure each operator was competent to safely operate the powered industrial truck. On or about 9 November 2013, an employee standing next to a powered industrial truck (forklift) while in operation and carrying/moving an unstable/unsecured load(slotted wall shelves) sustained injuries when the load shifted and the employee jumped in front of the unstable/unsecured load to stop the shifting load from falling.
