On or about 9 November 2013, an employee standing next to a powered industrial truck (forklift) while in operation and carrying/moving an unstable/unsecured load(slotted wall shelves) sustained injuries when the load shifted and the employee jumped in front of the unstable/unsecured load to stop the shifting load from falling.
