29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.        New District Superintendents Office, Fort Bragg, NC: On or about 11/20/2013, employees had no means of safe access and egress working from a frame scaffolding.

29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.   New District Superintendents Office, Fort Bragg, NC: On or about 11/20/2013, the employer did not develop and implement a safety & health program that addressed workplace hazards such as, but not limitied to inspecting a scaffold for visible defects and fall hazards.
