29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:   (a) Jobsite, Consad Bldg., 2nd Floor, 123 N. Highland Avenue, Pittsburgh, PA:  On or about 11/22/13 the outlet boxes used to provide temporary power did not have covers in place.   Employees had plugged in battery packs to use on site and were exposed to potential electrical shock hazards.
