29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: (a) Jobsite, Consad Building, 3rd Floor, 123 N. Highland Avenue, Pittsburgh, PA: On or about 11/22/13 the outlet boxes used to provide temporary power did not have covers in place. A flexible cord was plugged into one of the outlets and was being used to supply power to a chop saw. (b) Jobsite, Consad Building, 4th Floor, 123 N. Highland Avenue, Pittsburgh, PA: On or about 11/22/13 the outlet boxes used to provide temporary power did not have covers in place. Three flexible cords were plugged into the outlets and were being used to supply power to screw guns.

Highland Avenue, Pittsburgh, PA: On or about 11/22/13 the top rail height of a guardrail measured approximately 48 inches above the floor and no mid-rail was in place. Employees were working in this area installing metal framing exposing them to potential fall hazards of approximately 20 feet to the ground below.

Highland Avenue, Pittsburgh, PA: On or about 11/22/13 no mid-rails were installed between the floor and top edge of the guardrail system. Employees were working in this area pushing scrap to the ground below exposing them to a potential 10 foot fall hazard. (b) Jobsite, Consad Building, 3rd Floor, 123 N. Highland Avenue, Pittsburgh PA: On or about 11/22/13 no mid-rails were installed between the floor and top edge of the guardrail system. Employees were working in this area installing metal framing for the windows and were exposed to a potential fall hazard of 20 feet. (c) Jobsite, Consad Building, 4th Floor, 123 N. Highland Avenue, Pittsburgh PA: On or about 11/22/13 no mid-rails were installed between the floor and top edge of the guardrail system. Employees were working in this area installing metal framing for the windows and were exposed to a potential fall hazard of 30 feet.
