29 CFR 1910.268(g)(1): The employer did not ensure that safety belts and straps were provided and used when work is performed at postions more than 4 feet above the ground, on poles, and on towers:  On or about November 22, 2013, at the jobsite located at 441 E. 29th St. N., Wichita, KS:  An employee engaged in tower maintenance activities was exposed to fall hazards of approximately 60-80 feet to the ground below in that 100% fall protection was not maintained at all times.
