29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:     a) On November 13, 2013 and at times prior thereto; front of building; employees were using a table saw that was not properly guarded.        Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On November 13, 2013 and at times prior thereto; front of site, employees were operating a table saw which was not equipped with an anti-restart/magnetic switch to prevent it from accidental start up if the power was lost and re-engaged.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     On November 14, 2013 and at times prior thereto; Northside of residential home; employees working off a ladder-jack scaffold system were not protected from falling 15 feet to ground level by a personal fall arrest system.         Abatement certification and documentation required within 10 days after abatement date.
