29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a)  Rod Machining Area:  The employer did not ensure that a Yoshida drill press (model #YD2-54, serial #5521010) was equipped with machine guarding to protect employees from the rotating chuck and the tap/thread cleaning attachment, as determined on November 26, 2013.     (b)  Machine Shop:  The employer did not ensure that a Kira drill press (model #KRTG-5410, serial #58807) was equipped with machine guarding to protect employees from the rotating chuck and the unused portion of the tap or bit, as determined on November 26, 2013.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:      (a)  Machine Shop:  Employees operate a Baldor abrasive pedestal grinder (model #1216W, serial #P400) that had a maximum opening of one inch from the grinding wheel and the tongue guard, as determined on November 26, 2013.
