29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed.  Warehouse on or about 26 November 2013 and times prior thereto; the employer did not ensure that exit paths were unobstructed. Mobile ladder stands were located along the exit path, between storage shelves impeding egress. Two instances were noted:             (a) Warehouse, east center aisle.        (b) Warehouse, northeast side aisle, paralleling the northern wall.   This condition exposed employees to delays in exiting the building, smoke inhalation, fire, and tripping hazards.

Throughout the warehouse area, no emergency lighting is available. This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

Office area on or about 26 November 2013 and times prior thereto; two instances were noted where exit signs were not posted along the exit route.               (a) East exit adjacent to copying machine, exit route was not marked with an exit sign.         (b) South exit office area leading to break area, exit route was not marked with an exit sign.   This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

Warehouse on or about 26 November 2013 and times prior thereto; the double-doors situated on the west side of the exit route; that provides access to the arctic rollup door entry, was not marked "Not an Exit" or identified by signage marking its actual use. This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace.   Warehouse on or about 26 November 2013 and times prior thereto; the employer did not ensure that material safety data sheets for each hazardous chemical located in the workplace was readily accessible. A random sample of chemicals were selected. Four instances were noted:       (a) 3M Scotch-Gard protectant, 10 ounce.        (b) Krylon Fusion, super bond paint 12 ounce.        (c) Sem Shredder Oil, 64 ounce.       (d) WD-40 lubricant, 11 ounce.  This condition exposed employees to chemical burns, irritation of the eyes and upper respiratory tract, and skin dermatitis hazards.

29 CFR 1960.58:  The Agency did not provide training for collateral duty safety and health personnel including all members of certified occupational safety and health committees commensurate with the scope of their assigned responsibilities.  NARA Pacific Alaska Office on or about 26 November 2013 and times prior thereto; the employer did not provide training for appointed collateral duty safety and health officer to recognize basic safety and health hazards in the workplace and effectively assist agency safety and health officers with inspection and abatement efforts. This condition exposed employees to inadequate instruction, orientation and/or training.
