29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  Turning department, Yama Seiki GA-3600L turning machine and Haas EC400 HS-1 serial no. 50082 horizontal milling machine: on November 26, 2013, employees operating and setting up machines were exposed to amputation, caught-in, and struck-by hazards where safety interlocks were not operational or by-passed, and alternative methods of protection to prevent employees' access to the point of operation during production cycle and when working in the danger zone were not provided.  There is no abatement certification or documentation is required for this item.

There is no abatement certification or documentation required for this item.
