29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  1. In the Fabrication Shop, five (5) fourplex 110/120 volt metal outlet boxes with knockouts were being used as extension cords to energize equipment, tools and machinery used in the shop.The boxes were used as drops from the ceiling.  2. In the Fabrication Shop, eight (8) 240 volt metal outlet boxes with knockouts were being used as extension cords to energize equipment, tools and machinery used in the shop. The boxes were used as drops from the ceiling.

29 CFR 1910.303(f): Each service, feeder and branch circuit, at its disconnecting means or over current device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:(a) (LOCATION) (IDENTIFY EQUIPMENT) (STATE HAZARD(S) TO EMPLOYEE(S))  1. In the Final Assembly Shop, on the north wall near the washer and dryer, there was a 277/480 volt panel box with breakers inside that was not marked to indicate the purpose of each breaker contained inside of the box.  2. In the Fabrication Shop, on the north wall, there was a 240/480 volt panel box with breakers located inside that was not marked to indicate the purpose of each breaker housed inside of the box.  3. In the Fabrication Shop, on the south wall, there was a 240/480 volt panel box with no ledger in place to explain the purpose of each breaker situated inside of the box.  4. In the Fabrication Shop, on the east wall, there was a 240/480 volt breaker box with no ledger in place to indicate the purpose of each breaker located inside of the panel box.  5. In the Fabrication Shop, on the east wall, there was a 240 volt small panel box that was not marked to convey the purpose of each breaker residing inside of the box.
