29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. (a) 504 Lakeside Dr, Lee's Summit, MO 64064; No access ladder was used to access tubular welded scaffolds. Employees were exposed to fall hazards when climbing crossbraces and end panels of the scaffolds. End panels were not configured with uniform rung-like supports. (a) 504 Lakeside Dr, Lee's Summit, MO 64064; A step ladder was set on the top level of the scaffold over the driveway and leaned against the exterior of the house. The scaffold was not secured from being pushed away from the house. Scaffold planks were not secured to the scaffold. Ladder legs were not secured to prevent them from slipping off scaffold planks. An employee was applying EIFS from the closed step ladder and was exposed to a potential fall hazard. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
