29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems or personal fall arrest systems:  On or about November 25, 2013 at worksite 3534 McGee in Kansas City, Missouri- Employees were performing roofing operations at an elevated height on a steep pitch roof without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.   Abatement certification for this item is not required.

29 CFR 1926.1053(b)(1):   Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about November 25, 2013 at worksite 3534 McGee in Kansas City, Missouri- Employees were accessing a working surface on a steep pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.     Abatement certification for this item is not required

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about November 25, 2013, at a job site located at 3534 McGee in Kansas City, MO, workers were exposed to serious fall hazards in that they had not been trained in fall hazard recognition and the proper installation or use of the equipment, procedures and systems designed to mitigate those hazards.
