Rotating parts, such as the chuck and unused portion of the drill bit, on a Guardian Power Products Heavy Duty Drill Press [Model: DP-Mini (GP)] were not guarded to protect employees from hand injuries.    2. Employees were exposed to eye injury as a safety shield, intended to deflect flying metal chips during rotor cutting, was not installed on an Ammco Model 3000 Brake Lathe (SN 47418).
