29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section: This violation was discovered on or about November 27, 2013, where employees, who maintained and serviced equipment and machinery such as but not limited to shears, benders and band saws throughout the facility that had multiple sources of energy, did not utilize energy control procedures to prevent the unexpected energizing, startup or release of stored energy which could cause injury.

This violation was discovered on or about November 27, 2013, where employees, who operated a Krando overhead crane were exposed to struck-by hazards due to the missing (inoperative) emergency stop botton on the pendent control.
