29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept in a sanitary condition:    On or about November 22, 2013:  Employees working in the facility located at 6369 Peachtree Street are exposed to surface contact hazards from the accumulation of contaminants such as, but not limited to iron oxide.  Surface wipe sampling was conducted on November, 22 and December 12, 2013.  These areas had the following accumulation of iron oxide per 100 cm2:      (a)  Sample #13-42, Top of file cabinet in Lab Supervisor's Office:  388.11 micrograms      (b)  Sample #13-36, On top of refrigerator in the break-room:  286.91 micrograms   (c)  Sample #13-34, Top right side of stereo in break-room:  276.84 micrograms   (d)  Sample #13-39, Workstation tabletop near the blending area:  179.46 micrograms   (e)  Sample #13-40, Workstation in lower level of blending area:  134.94 micrograms   (f)  Sample #13-38, Top right side of file cabinet in S&R office:  122.69 micrograms   (g)  Sample #13-41, Tabletop near the weigh scale in lab:  116.20 micrograms  (h)  Sample #13-35,  On top of counter in front of small microwave: 96.79 micrograms

29 CFR 1910.1027(f)(2)(i): When the permissible exposure limit (PEL) was exceeded, the employer did not establish and implement a written compliance program to reduce employee exposure to cadmium to or below the PEL by means of engineering and work practice controls:    On or about November 22, 2013:  Employees working in the blending area are exposed to air contaminants such as, but not limited to cadmium.  Air sampling was conducted on employees December 12, 2013.  One employee was over the cadmium permissible exposure limit (PEL) of 5 micrograms per cubic meter of air by more than nine times the limit.  The employer had not established and implemented within their written program, engineering and work practice procedures to reduce employee expose to cadmium.

29 CFR 1910.1027(j)(1): The employer did not provide clean change rooms, hand washing facilities, showers, and lunchroom facilities that complied with 29 CFR 1910.141 for employees whose airborne exposure to cadmium was above the permissible exposure limit (PEL):   On or about November 22, 2013: Employees working in the blending area are exposed to air contaminants such as, but not limited to cadmium.  Air sampling was conducted on employees December 12, 2013 and the employee conducting the cadmium blending process was exposed at an 8 hour time weighted average (8hr TWA) of 46 micrograms per cubic meter of air, which is more than nine times the permissible exposure limit.  The employer does not provide a change room for employees conducting this process.

The employee conducting cadmium blending was over the cadmium permissible exposure limit (PEL) of 5 micrograms per cubic meter of air, by more than nine times the limit.  The employer did not require employees conducting cadmium blending to shower at the end of their work shift.

Employees 3M half face respirators are not thoroughly cleaned and voluntary use is not addressed in the respiratory protection program.
