29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Location - Production Area:  On or about December 10, 2013, a Guifil PE 30-100 press brake was not guarded at the ppoint of operation.    (b) Location - Production Area:  On or about December 10, 2013, the headstocks on a LeBlond mill and a Drive All mill were not guarded.    (c) Location - Dust Room:  On or about December 10, 2013, the blade of a table saw used to cut ceramic furnace parts was not guarded.    (d) Location - Dust Room:  On or about December 10, 2013, the blade of a band saw used to cut ceramic furnace parts was not properly guarded.

Location - Welding Area:  On or about December 10, 2013, a knockout was missing from the box at the terminal end of an extension cord that was used to power an electric welder.
