Site- Employees taking boats in and out of the water with a forklift were not protected from crushing injuries where the forklifts are driven up to the boat launch and there is not an adequate means of stopping the forklift from driving off the boat launch. On or about November 22, 2013 an employee was exposed to a crushing hazard when the Taylor, model TM-90S, forklift they were operating was driven off the boat launch and fell into Lake Winnipesaukee.
