29 CFR 1910.1026(j)(1)(ii): The employer did not ensure that all spills and releases of chromium (VI)-containing material were cleaned up promptly:   At the facility, at the catalyst activation unit:  On or about December 3, 2013, spilled or released chromium (VI) containing material was not cleaned from structural beams and work platform surfaces, exposing employees to inhalation and chemical eye and skin contact hazards.

29 CFR 1910.1026(k)(1)(i)(C): The employer did not make medical surveillance available for all employees who were exposed to chromium (VI) in an emergency:   At the facility, at the catalyst activation unit:  On or about November 4, 2013, medical monitoring was not provided to exposed employees when an uncontrolled discharge of chromium (VI) containing material from the catalyst activation unit stack occurred, exposing employees to an inhalation hazard.
