OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of serious crushing injuries:  (a) Location - Service: On or about 12/4/13, employees working with the Bend Pak and Rotary Lifts were exposed to crushing hazards.  Operators were not trained in the safe use and operation of the lifts and the employer failed to perform periodic inspections.
