29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction: a) Grease pit The large circular opening to the grease pit, approximately 30 inches in diameter, was loosely covered by an unsecured, thin steel cap. On 11/19/2013 an employee fell into the outdoor grease pit and had to be rescued by kitchen workers.
