FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

As such, the ladder was approximately 3 feet 11 inches too short from reaching the upper walking/working surface.  Condition observed on or about 11/08/13.
