29 CFR 1926.300(b)(1):Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  (a) Crystal Pond Rd at Madison Place Shrewsbury, MA: On or about December 4, 2013, an employee was observed operating a portable Bosch 10 inch table saw without the manufacturer supplied blade guard in place, which is not in accordance with this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.403(b)(2):Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  (a) Crystal Pond Rd at Madison Place Shrewsbury, MA: On or about December 4, 2013, employees were observed operating numerous power tools that were powered through a Defiant GFCI which was plugged into an extension cord which is contrary to the instructions printed on the device and is not in accordance with this subpart.
