29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a). Worksite located at 11700 E. Highway 24, Independence, Missouri- Employee was exposed to overhead hazard from equipment and tools located overhead as well as striking his head on the top of the work location he had positioned himself.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (or in the alternative):     29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:     (a). Worksite located at 11700 E. Highway 24, Independence, Missouri- Employee was exposed to a fall hazard in that the employee had climbed in to and out of a wall opening using a ladder that did extend at least 3 feet above this opening.
