29 CFR 1910.305(f)(1):  All conductors used for general wiring were not insulated unless otherwise permitted in this subpart:  (a) D Press:  Employees were exposed to electrical hazards as the electrical cord to the Baldwin Stack-o-veyor for D Press for Folder Delivery did not have adequate insulation as the insulation was damaged.  (b) D Press by Emergency Eyewash:  Employees were exposed to electrical hazards as the electrical cord for the Shop Vac did not have adequate insulation as the insulation was damaged.

(b) B Press:  Employees were exposed to electrical hazards as there was not adequate strain relief to the cord where it connects to the Ink Level Control Box for Repress Unit 3.
