Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard from flying polyvinyl chloride (PVC) pipe fragments:    (a) Prevo, 26 Cocalico Creek Road, Ephrata, Pennsylvania - The employer used schedule 40 polyvinyl chloride (PVC) piping of varying diameters to transport compressed air to pneumatically powered equipment and machinery, exposing employees to a struck-by hazard from flying PVC pipe fragments associated with such use of PVC piping in above ground installations.  According to the Plastic Pipe Institute and the Plastic Pipe and Fittings Association, PVC piping does not possess shatter resistant properties, and failure of the piping presents a serious hazard to employees from resultant flying shards or pipe fragments which become rapidly propelled by the released energy of a suddenly decompressing gas, on or about December 5, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

OSH ACT of 1970 Section (5)(a)(1): 29 CFR Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards from combustible wood dust:    (a) Prevo, 26 Cocalico Creek Road, Ephrata, Pennsylvania - The dust collection system, used to collect Class II wood dust from saws, sanders, planers, shapers and other woodworking equipment had the following deficiencies:  (1) the system recycled exhaust air back into the building and the cyclone dust collector did not have protection to prevent fire and explosion in the cyclone from propagating into the building through the recycled air duct; and (2) the ductwork conveying the Class II wood dust from the woodworking equipment to the cyclone dust collector did not have isolation devices to prevent fires and deflagrations from propagating from the dust collector back through the inlet ductwork to the woodworking equipment, exposing employees to fire and explosion hazards, on or about December 5, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
