OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of falling into elevator hoistway without a vision panel installed on the hoistway doors of a manually operated elevator to determine the elevator car's position.     a) Ground floor lobby, manually operated elevator:     A vision panel was not provided on the manually operated hoistway doors of horizontally sliding type for a manually operated elevator with continuous-pressure operation. An employee could not determine the position of the elevator car through the hoistway doors without a vision panel when he opened the hoistway doors, and fell approximately 18 feet from the ground floor through the elevator hoistway to the elevator pit.
