29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Garage Area: A metalic sheathed cable installed from the light and draped over the garage door was strung loosly to the front wall of the garage near the walk-in door. The cable did not closely follow the surface of the building and was not supported and secured by staples, cable ties, straps, hangers, or similar fiittings designed and installed so as not to damage the cable, at intervals not exceeding 4 1/2 feet and withing 12 inches of every outlet box, junction box, cabinet, or fitting. The cable was not protected from damage, exposing employees to potential electrical hazards.     Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.304(b)(2): Outlet devices had an ampere rating less than the load being served:    (a) The GE widow air conditioning unit in the cold dry storage room and the commercial toaster oven in the kitchen were plugged into the same circuit. When both pieces of equipment operated simultaneously, the breaker would trip. The circuit was overloaded exposing employees to electrical hazards.      A licensed electrician was called in during the inspection and found that the window air conditioning unit needed it's own circuit and that the circuit ithe window air conditioner currently pluggs into is overloaded when the toaster oven and window unit both pull amps.    Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    (a) Two junction boxes in the garage near the walk-in door had openings that exposed employees to potential contact to live electrical parts. One box had a duplex receptacle in it; the other had a switch in it. Employees were exposed to electrical hazards. Employees were exposed to electrical hazards. 29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:     (a) The Crewman 2000 floor buffer, stored in a closet next to the clean laundry, had a damaged electrical cord which was partially taped with electrical tape. Inner insulated wires were still visible. Electrical tape will not return a damaged cord to its original insulated condition. Employees were exposed to electrical hazards.      This violation was abated during inspection. Abatement certification for this violation is not required.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labeling system and the material safety data sheet, and how employees can obtain and use the appropriate hazard information:    (a) Training has not been updated as required under 1910.1200(j)(1).   - Employees were not trained on the new label elements in paragraph (f) and Appendix C of 29 CFR 1910.1200 by the effective date of December 1, 2013.   - Employees were not trained on the new Safety Data Sheet format as described in paragraph (g) and Appendix D of 29 CFR 1910.1200 by the effective date of December 1, 2013.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
