29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a) At 850 Bollweevil Circle Enterprise, Alabama: On or about December 2, 2013 and at times prior; the employer failed to ensure his employees were protected from impailment hazards in that employees were allowed to walk and work around protrouding reinforcing steel that was not guarded to elimnate the hazard of impalement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a) At the South Side Wall: On or about December 2, 2013 and at times prior; the employer allowed his employees to use a portable extension ladder to gain access to and from the scaffolding and the ladder did not extend at least three feet above the landing area exposing employees to falls of 13.1 feet to the ground below. b) At the West Side Wall: On or about December 2, 2013 and at times prior; the employer allowed his employees to use a portable extension ladder to gain access to and from the scaffolding and the ladder did not extend at least three feet above the landing area exposing employees to falls of 13.1 feet to the ground below.
