On or about December 6, 2013, Production Area:  The employer had not conducted an assessment of the workplace to determine the need for personal protective equipment.  Among other hazards, employees in the shop were exposed to dust and flying particles from grinding operations.

29 CFR 1910.215(d)(1): Spindle speed(s) on grinding machine(s) were not checked before mounting to assure that it did not exceed the maximum operating speed marked on the wheel:   A.  On or about December 6, 2013, Shop:  A Tru-Maxx 8" diameter aluminum oxide vitrified abrasive wheel, marked with a maximum operating speed of 3105 rpm, was mounted on the Gray way grinder, which rotated at 3600 rpm.
