As observed on or about January 16, 2014, at the above addressed jobsite, an extension cord posed a tripping hazard. The cord extened from an outer wall of the office to the work area.

The stairway lead from the ground level to an area above the office.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  As determined on or about January 16, 2014, at the above addressed jobsite, the employer had not implemented a written hazard communication program for the employees who are exposed to the chemicals related to manufacture of stone counter tops. These chemicals include, at least, the dust of the stone, the adhesives and the solvents related to the fabrication process of counter tops.
