29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Exterior Front Entrance  - Employees were observed standing on a steel canopy that measured 3 ft. wide by 5 ft - 1inch long,  drilling holes into brick mortar joints and exposed to a 9 ft. 8 inch fall hazard, on or about 12/05/2013.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Exterior Front Entrance - Employees were not provided fall hazard recognition training as evidenced by employees standing on a 3 ft. wide by 5 ft. 1 inch long steel canopy  drilling holes into a brick mortar joint and exposed to a 9 ft. 8 inch fall hazard, on or about 12/05/2013.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when an extension is not possible because of the ladder's length,  the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. In     a) Exterior Front Entrance - Employees were observed using an 8 ft. portable step ladder to gain access to the top of a steel canopy.  The top of the ladder was 25 inches below the landing surface, on or about 12/05/2013.
