29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    (a)        Western Stone Company of Colorado, 4278 Ute Highway, Lyons, CO 80540: On and before December 2, 2013, the forklift was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This condition exposes employees to tipping hazards.    Abatement Note: Abatement certification and documentation are required for this item (See enclosed Sample Abatement Certification Letter).

Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").     Abatement Note:  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

This forklift was originally equipped with a seatbelt according to the manufacturer.
