29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about December 10, 2013, at the above addressed site, an employee was exposed to a fall hazard of approximately 12 feet 3 inches when performing steel work from a work platform supported by the forks of a rough terrain fork lift that was not specifically designed by the manufacturer for such use.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: On December 10, 2013, at the above addressed site, an employee was exposed to a fall hazard of approximat6ely 12 feet 3 inches while performing steel work from a scaffold platform supported by a rough terrain forklift while not wearing a lanyard attached to his safety harness and the platform guardrail/door was completely ajar.
