OSH ACT of 1970 Section (5)(a)(1): An employee was exposed falls from ladders set up on snow and ice covered ground:     a)      On December 10, 2013, to access a rooftop HVAC unit an employee used an unsecured fiberglass extension ladder placed on a snow and ice covered surface without suitable means to prevent slipping.    Note: Abatement certification AND documentation are required for this item.
