Employees walking and working beneath the scaffolding were exposed to being struck by dropped equipment.

At this workplace an employee installing framing material on an exterior wall of a two story single family resident worked from a scaffold at the height of more than twelve feet without anchor points for personal fall arrest harness tie-off. Employees were exposed to a fall hazard greater than 6 feet.
