29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.       On or about December 11, 2013, on the west side of the construction site, employees worked from a JLG lift work platform without wearing a personal body harness and lanyard attached to the basket to prevent a fall to the ground of approximately 16 feet.

29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position.   On or about December 11, 2013, on the west side of the construction site, employees working from a 6015 JLG lift were performing steel erecting activities while standing on the mid-rails of the basket.  The employees were exposed to fall hazards of approximately 16 feet.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.         On or about December 11, 2013, on the west side of the construction site, employees engaged in steel erecting utilized an extension cord that was missing the ground pin, exposing employees to electrical hazards.
