29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:   a. Wooden Stairway Leading from Ground Level to the First Landing:  The enclosed wooden steps used by an employee consisted of 5 treads which measured approximately 45 inches wide by 11.5 inches deep and had a rise of approximately 7.25 inches between treads were not equipped with at least one hand rail as observed on 12-10-13.   b. Wooden Stairway Leading from First Landing to the Second Floor Level: The enclosed wooden steps used by an employee consisted of 21 treads which measured approximately 45 inches wide by 11.5 inches deep and had a rise of approximately 7.25 inches between treads were not equipped with a least one hand rail as observed on 12-10-13.   c. Wooden Stairway Leading from Second Level to the Landing Prior to the Third Floor: The enclosed wooden steps used by an employee consisted of 13 treads which measured approximately 45 inches wide by 11.5 inches deep and had a rise of 7.25 inches between treads were not not equipped with at least one hand rail as observed on 12-10-13.

The facility's Office Manager, not a company executive signed the summary, as determined on 12-12-13.      b. 2011 OSHA Form 300A:  The 2011 OSHA Form 300A summary of work related injuries and illness was not certified by the highest ranking company executive at the facility.  The facility's Office Manager, not a company executive signed the summary, as determined on 12-12-13.      c. 2010 OSHA Form 300A:  The 2010 OSHA Form 300A summary of work related injuries and illness was not certified by the highest ranking company executive at the facility.  The facility's Office Manager, not a company executive signed the summary, as determined on 12-12 2013.      Abatement Note:  Proper certification of the Summary of Work-Related Injuries and Illnesses (OSHA Form 300A) requires a company executive to examine and determine based on their knowledge that the information was recorded correctly.  A company executive includes; owners, officers, and the highest ranking company official working at the establishment.

Renovations of Mason County Sheriff's Office: The employer did not train his employee who work with hazardous materials such as but not limited to curing compound,gasoline, diesel fuel and kerosene in the new label elements such as but not limited to the new pictograms, signal words and the new safety data sheet format as determined on 12-11-13.
