29 CFR 1910.157(e)(3): The date of the annual maintenance check of portable fire extinguishers was not recorded: On or about December 12, 2013, in the shop, the employer did not assure portable fire extinguishers on the west wall and in utility room were not labeled to indicate the date of inspection and inspection records were not made available to the OSHA compliance officer upon request. This condition exposed employees to delay in fighting incipient stage fire hazards.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating: On or about December 12, 2013, at the shop located on the east wall, the employer did not assure that acetylene and oxygen cylinders were not stored side by side. Acetylene and oxygen cylinders were stored side by side; less than 20 feet (6.1 m) without any form of one-half hour, fire-resistant, 5 feet (1.5 m) barrier. This condition exposed employees to the hazards of being burned in the event of an ignition source or fire.

This created an electrical shock, burn and fire hazard.
