29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:     (a) Jobsite: 10 West North Avenue, Pittsburgh, PA 15222, Near Front Entrance: On or before November 4, 2013, employees were exposed to electrical and/or fire hazards in that a four inch square metal outlet box used for lighting control was found hanging by its cables.     (b) Jobsite: 10 West North Avenue, Pittsburgh, PA 15222, Near Front Entrance: On or before November 4, 2013, employees were exposed to electrical and/or fire hazards in that a single gang metal outlet box was not mounted. The receptacle was powering a quartz light.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations:     (a) Jobsite: 10 West North Avenue, Pittsburgh, PA 15222, Structural Integrity: On or before November 4, 2013, employees were exposed to crushing type hazards in that an engineering survey was not completed prior to beginning internal demolition operations such as ceiling removal.
