29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews:  a) On or about December 3, 2013 - at the jobsite located at 520 W. 138th Street, Chicago, IL. - Employees were performing masonry activities on a tubular welded frame scaffolding that was not completely guarded along all sides and edges, thereby exposing the employees to falls of approximately thirty (30) feet.
