Section 5(a)(1) of the Occupational Safety and Health Act of 1970:   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to being struck by due to failure of wire rope within the hoist drum. Implement company policy for equipment inspection before use, as ASME B30.7-2011, Chapter 7-2 Inspection, Testing, and Maintenance, Section 7-2.4.2. Rope Inspection (a).  B. Implement company policy for equipment replacement as ASME B30.7-2011, Chapter 7-2 Inspection, Testing, and Maintenance, Section 7-2.4.2. Rope Replacement (b). (4).
