29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:  On or about December 13, 2013, at Infiniti of Duluth, 3090 Satellite Blvd, Duluth, Georgia:  Three employees performing framing work were exposed to a mild electric shock hazard when they used a Dewalt D28715 Chop Saw with a damaged cord that had been spliced together side by side and held together with tape.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about December 13, 2013, at Infiniti of Duluth, 3090 Satellite Blvd., Duluth, Georgia:  Two employees performing framing work from the basket of a JLG 400S, number 679988 aerial lift were exposed to a fall hazard of approximately 17-feet when they did not wear a harness and tie off to the basket of the lift.
