29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a)  At 27749 Rileywood Dr Daphne, Alabama:  On or about December 18, 2013 and at times prior; the employer failed to ensure an employee operating a vehicle mounted bucket had a body belt or harness on that was attached to the bucket exposing the employee to fall hazards of 50 feet to the ground below.
