29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:     On or about December 17, 2013, at the job site located at 1304 North Pine Street, North Little Rock, AR 72114, on an articulating lift, the employer did not ensure employees stand firmly on the floor of the basket. Employees were observed walking down the slope of the roof and climbing over the top rail of the aerial lifts basket to descend to the ground below. This exposed employees to fall hazards of greater than 34 feet to unfinished ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On or about December 17, 2013, at a job site located at 1304 N. Pine Street, North Little Rock, AR 72114, on a steep roof, the employer did not ensure employees working at heights greater than 34 feet above the unfinished ground used fall protection. Employees on the west side of a steep roof installing roofing insulation, harnesses were not attached to a rope grab device or a vertical lifeline that was attached to an anchor point on the roof. This exposed the employees to fall hazards of greater than 34 feet to the unfinished ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:        The TNT CONTRACTING AND ERECTING, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 315174342, citation number 1, item number 1 and was affirmed as a final order on April 13, 2011, with respect to a workplace located at, 548 Brookhill Ranch Road, Hot Springs, Arkansas 71901.     On or about December 17, 2013, at a job site located at 1304 n. Pine Street, North Little Rock, AR 72114, on an articulating lift, the employer did not ensure an employee's personal fall arrest system lanyard was attached to the aerial lift. An employee was observed in aerial lift without his personal fall arrest system lanyard anchored to the boom or basket. This exposed employees to fall hazards of greater than 34 feet to unfinished ground below.
