29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a)  On 12/18/2013, in the 14200 block of John Day Drive, Haslet, Texas, employees had not been trained to recognize the hazards and methods to prevent being struck by vehicular traffic in work zones, such as, but not limited to, the use of warning signs, devices and barriers.

29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards:    (a) On or about 12/18/2013, in the 14200 block of John Day Drive, Haslet, Texas, where the employer did not ensure advance warning signs and devices had been used to warn oncoming traffic of work activity, thereby exposing employees in the work zone to struck-by hazards.

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA    (a)  On or about 12/18/2013, at 14200 John Day Road, Haslet, Texas, employees were exposed to being struck by oncoming public traffic. The employer failed to ensure adequate warning devices were utilized.
