29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) On 12/18/2013, in the 14200 block of John Day Drive, Haslet, Texas, employees had not been trained to recognize the hazards and methods to prevent being struck by vehicular traffic in work zones, such as, but not limited to, the use of warning signs, devices and barriers.

The employer failed to ensure adequate warning devices were utilized.
