29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:     a)      On or about December 16, 2013, at the above addressed jobsite, an employee was exposed to shock and electrocution hazards while operating a Rigid pipe threading machine, that had a 110 VAC electrical cord attached to the switch pedal that had that had missing / damaged outer jacket insulation that had not been visually inspected.
